Your Individual Plan Review
Once you are enrolled in both Medicare A & B, you are eligible to move forward with either a Medicare Advantage or Medicare Supplement plan. At this time you are ready to schedule your plan review with Carrie Hagel to discuss your options.
During your plan review, Carrie will determine which plans are available to you (based on the county that you reside in). From there, she will ensure that your healthcare providers are in the network of the plan you wish to enroll in and, if you'd like, she'll review your prescription medications to help determine any out-of-pocket expenses.
Important Items to Have Available at Your Plan Review:
Please have the following information available at your plan review appointment:
- • Your Medicare ID Number (found on your red, white, and blue Medicare card)
- • A list of your healthcare providers
- • A list of your prescription medications
If you are on Medicaid, please have your Medicaid number available.
If you have Tricare For Life or VA benefits, please notify Carrie as well.
